No: Transitions are difficult but those with specialized issues are often more affected by this quandary. The Pediatric sub-specialties like electrophysiology will often see young adults well into their twenties. You need to ask your treating team about how/when you will move to a different team. They have been through the process & often have suggestions on which people are recommended.
