Insomnia: Insomnia is a very prevalent condition. Sleep habits, stress & diseases can cause it. Get medical evaluation.

Sleep: Possible causes of insomnia include: poor sleep hygiene,Traveling with change in time zones, chronic pain, frequent night-time urination, problems breathing, Shift work, anxiety disorders (esp. PTSD), Depression, bipolar disorder, stress, medications (i.e. stimulants), caffeine, alcohol, eating large amts before bed, aging, restless leg syndrome, sleep apnea, etc.

A number of things: There could be sleep apnea. You could also have underlying depression as well, which can also disturb sleep. Caffeine may also disturb sleep, as can alcohol. You should definitely see a sleep specialist to go over all of your sleep habits and complaints to chart out a management plan. You may also need a sleep study in a hospital or sleep lab.
