Impairment /Severity: You definitely need help if depression or anxiety are causing significant impairment in your quality of life. You also definitely need help if you are feeling the symptoms are severe or you are feeling desperate or suicidal.

Distress: Most people go to the doctor when they're hurting in some way. Emotional pain is real, and it can be manifested in ways that interfere with a person's overall functioning. A person may suffer from inner turmoil causing distress and uneasiness. Often, a person's sleep patterns and appetite will change. It's good (and not a sign of weakness), to try and get some help sooner, rather than later.
