Less free estrogen: If the shbg is high, then the amount of "free" or "active circulating estrogen or testosterone is low.

Insulin resistance: Usually pcos patients have low shbg, which causes the bio available testosterone levels to increase. High free testosterone causes problems even when the total level is normal. If a pcos patient's shbg level goes up this is most often seen when ocp's are used. Weight loss and metformin may help, but not to the degree of ocp.
