Usually not: In toeing or being "pigeon toed" is usually just a variation on anatomy, just like some folks have high arches, some people are flat footed and everyone else is in between. In toeing can occur with changes in the hip called femoral ante version, or in the leg bones called tibial torsion, or come from the feet called metatarsus adductus, or it can occur as various combinations of these changes.

When its' a problem: Occasionally the in toeing is so severe, (usually on one side) that it affects the ability to walk or play sports. Fortunately this is very rare, and requires surgical correction in childhood.
