Immunization records: Try calling your health department. I am uncertain about texas, but in new york, all vaccines given to a child is reported via computer to the state.

Hard to say: Your child's doctor is requires to keep such records until they reach 21 yo and have not been seen for 7 yrs. The local county medical society or Texas Medical Board can locate the doc if you can't. They may charge a fee but do keep them. If you signed up for the Texas immunization tracking program IMTRAC they would be recorded on the state system & retrievable by the kids identifying data.

Registry: If you signed up for the immtrac registry in texas the material is on file & can be downloaded by any doctor, school or day care center with a registry access code. Physicians are required to provide you with access to such records & send them within 15 business days after they receive a properly executed release & payment of any required fee.
