A : A board certified oral and maxillofacial surgeon, in addition to an ent, would be a good choice since they can evaluate issues within the mouth and upper throat. Burning mouth syndrome is usually due to issues within the oral cavity, but discomfort can extend down the throat.The fact that you have a hiatal hernia and are being treated for reflux is a significant clue to the problem. Even though you may not have symptoms of reflux, it still may be occurring. All of your symptoms can be explained by this.

You : You may very well have several things going on. Hiatal hernia, increased anxiety levels, and throat discomfort cerainly can be tied in with gerd or lpr. Many patients are silent refluxers, and the absence of classic heartburn complaints does not rule out clinically significant gerd or LPR which can affect vocal quality (hoarseness). Excessive dryness of the mouth can be associated with a clinical entity called " burning mouth syndrome" and can have some association with reflux disease. Therapy is generally directed at optimizing control of the reflux disease and symptomatic management of the dry mouth often worsened by high anxiety states. A recent study out of germany in the ENT literature showed clinical benefit with low-dose Klonopin (0.5 mg/day) for burning mouth syndrome. This could be discussed with your physician. Simple otc therapies for dry mouth include adequate hydration, biotene products ( mouthwash, toothpaste, topical gel), stoppers spray, and many other options which could be discussed. Prescription drugs include salagen, evoxac, (cevimeline) and others. I think there would be value in getting your anxiety under control as it is probably contibuting to both issues.
