A hiatal hernia can get stuck in your chest ( incarcerated) and strangulation is a rare complication. Straining such as heavy lifting, bearing down, coughing excessively does aggravate a hiatal hernia & can lead to complications. Strangulation means when the blood supply is cut off. that is unheard of in hiatal hernia. Sop straining, eat light meals, sleep with a slight head-up incline. Pl see gastroenterologist. I think some of your symptoms are from anxiety. So relax with proper care you should be fine.
