Good : Good morning. It isn't possible to tell you if you need surgery without a thorough assessment. It is probably a good sign if you don't have any swelling or bruising. Sometimes a strained ligament can be very painful. Sometimes people don't realize how much a sprain or a strain can hurt. If it isn't improving over the next few days you could consider seeing your family doctor or a podiatrist.

Just : Just to add my two cents to dr. Fowler's excellent answer: you may have a fracture, which can only be determined with an x-ray. Any injury to a ligament, including a tear, can only be seen on an mri. You really do need to get checked. Walking, or even an attempt to walk on an ankle fracture or torn ligament can make a bad situation much worse. Until you're seen, rest, ice, elevation, and until you know for certain what's wrong, use crutches and don't put any pressure on your foot. Good luck!
