A : A standard septoplasty shouldn't lead to any external bruising. All the work is done inside the nose.

The : The bruising around your eyes from a punch was probably because you sustained some degree of nasal fracture. With an endonasal septoplasty bruising externally shouldn't occur. If an external (open) rhinoplasty approach is taken (rarely employed for septoplasty alone unless the anterior cartilage is severely deformed), or conventional septoplasty is combined with rhinoplasty and the bones are broken, external bruising can occur. Internal bruising underneath the upper lip can also occasionally occur after septolasty if work is being done anteriorly on the boney crest that the septum rests on (nasal spine).
