Should be OK: Might have issues with prolonged sitting, but other than symptoms, doubt any harm to occur from your trip.

OK, but walk.: This shouldn't be a major concern. However i would instruct the patient to make sure they are regularly getting up and walking up and down the isle every now and again every 2 hours or so. A walk to the front of the bus then to the back and perhaps stay standing at their chair for a few minutes before sitting down.
