If an mri shows infarcts as well as periventricular ischemic changes does this mean a person has had an actual stroke rather than just a tia?

Yes: Tias do not show up on mris because the decreased blood flow (ischemia) is not severe enough and doesn't last long enough to cause a stroke (infarct).
