Possible hernia: Possibly, or an undescended testicle. He needs to see a pediatric urologist.

Could be!: I would probably visit your md and just consult about intermittent pain and retractile testis vs inguinal hernia ... Torsion or strangulation is very worrisome ! laparoscopic surgery is least invasive and healing is remarkable! but first a quick visit to your primary doc is necessary for a surgical referral! regards!
