Pain/function/shoes: If you have prolonged pain, limited function or the inability to participate in activities you enjoy or work you do, cannot wear shoes that you want/need to are common reasons. You may want to try some form of conservative care first to see if this can prevent surgery. Howver, the only way to correct the deformity is with surgery.

Personal decision: Possible reasons that people will opt to have surgical management for their bunions may include: pain due to pressure from ill fitting shoes, joint arthritis, inflammation of the joint capsule or bursa, abnormality of joint alignment, and nerve damage.

Daily problems.: You can never get your original foot back with surgery, but if your feet are causing you to change your activities of daily living, or giving you pain on a daily basis, it's time to consider surgery.
