University Setting: This is a very complicated situation. I'd suggest bringing her to a plastic surgery clinic at a local university. Things from special garments to surgery will be considered.

Difficult problem: This is essentially a problem of the lymphatic system. In rare instances this may be caused by parasites and should be evaluated. In most lymphatic compression and support may help. In more severe cases surgery may be warranted to devulk the extremity.

Elephantiasis: This is result of chronic lymphedema. The lesions are secondary to the increased pressure and swelling of the lower extremities. Use of compression stockings with at least a grade of 30–40 MM Hg would be recommended. Adjunctive to this possible treatment with manual lymphatic decongestion (MLD), or a lymphedema pump. This is a chronic condition and should be dealt with rapidly.
