What could cause painful muscle spasms in the foot and ankle area?

Muscle imbalance: There are many muscles, tendons and joints in the lower leg going into the foot, imbalance of any of these muscle groups could lead to spasm, as well as overuse syndromes. You cannot overlook the possibility of lower back problems as well as electrolyte imbalances. Consult your podiatrist or physician if simple remedies like Aspirin or tylenol (acetaminophen) do not give relief.
