Sometimes.: Irregular gestational sac is typically seen with anembryonic gestations (blighted ovum). It can also be seen with normal early pregnancies though. The more important aspect of gestational sac is its size (embryo and yolk sac should be visible once it is >2.5-3 cm) and location (ectopic pregnancy if outside the endometrial cavity). Serial beta HCG testing and ultrasounds can help with diagnosis.
