Many reasons: Cancer is the clearest reason, but there are a number of other problems for which conservative attempts may not work successively. Examples would be very large and symptomatic fibroids, excessive bleeding not responding to other treatments, endometriosis and pelvic pain unresponsive to conservative treatments, adenomyosis, or high risk of cancer and pt preference of surgery to screening.
