When a patient has metabolic acidosis from prolong diarrhea, what has happened in the gut?

Several possible: Diarrheic stools cause the body to lose bicarbonate, (sodium bicarbonate) and if sufficiently severe this will create metabolic acidosis. If the diarrhea is a manifestation of systemic disease this can produce a metabolic anion-gap acidosis. In either event a doctor should be seen asap.
