Rib dysfunction?: You say it's 'behind' the ribs, but imaging is neg. When ribs become stuck & don't move as they should, the restriction can cause pain. You can also get spasms in the intercostal muscles between the ribs.Relying on imaging is a poor substitute for a good physical. See an osteopathic dr for a good physical, incl. Hands-on exam and you have a better chance of finding both cause & proper treatment.

Possibly Nerve Rlted: Negative imaging with severe pain in the flank may be due to irritation of the intercostal nerve that runs on the undersurface of the rib. Other potential common causes for nerve like pain in that area if post-herpetic neuralgia which is pain after healing of shingles rash. Feelings of pressure without palpable mass can occur with nerve related pain.
