PA: It likely means that his office / business is listed as a Professional Association which allows certain differences from a legal perspective. It could also mean Physician Assistant although Physician Assistants are not "Dr"s unless they possess a PHD as well. Hope this is helpful.

Dr vs PA: Hi, its hard to know from your question, but i'll try. A PA is a Physician's Assistant. It has its own training program that resembles basic sciences in medical school, and has hands on clinical training before they can work independently. PA's can be doctors if they attain a PhD (doctorate of philosophy). Usually, there is a doctor, somewhere, taking responsibility for a PA in medicine.
