Depends: Joints are fused by removing the cartilage and placing the two raw bone ends together and fixating with screws and/or a plate. There are external fixators that can be used as well.

See below: In a simplified answer, the cartillage is removed from the ends of the bones to be fused and then the bones are fixated together with either a screw, screw and plate. This prevents motion of the bone so that it can heal.
