Can an enlarged liver be seen with the naked eye? or is a scan needed?

Palpation: An enlarged liver is usually felt by palpation of the right upper quadrant of the abdomen. For a liver large enough to be seen, it would have to be spectacularly large and you would probably have a host of other symptoms in that case. Scans can help narrow down the cause, once the enlargement is detected by the examining MD.
