Bleeding: Since its a focal area its more likely to be adenomyosis, which typically gets progressively worse as you age. However, you're also at the age when ovarian dysfunction often causes irregular, heavy and/or prolonged bleeding. You might be a good candidate for an endometrial ablation assuming you are done with having babies.
