Could be Lyme: it could be Lyme disease and doxycycline helps that but usually we don't see that rash this time of year. You need to see a dermatologist to get this clarified. This could be erythema multiforme.

Exact location?: One of the conditions that can affect the foot joints with swelling and redness is gout, a condn. where there is excess uric acid gets deposited in the form of salt in the cartilages and the surrounding soft tissues of the area affected.
