Not just for Ca: Prostate surgery done not just for cancer , most commonly done foe benign enlargement that causes obstruction , only enlarged part ( adenoma ) is removed ,prostate gland itself left behind , unlike in cancer the entire gland will be removed. On your dad the scar formation or recurrence possibly the cause.

No.The removal was: likely to help him pee more freely.The tissue removed helps open up the urinary tract so one doesn't stop/start( indicative of blockage of urinalysis tract).This is all speculation on what little info you wrote.I hope it helps. Sorry for your loss.
