If there is a: hernia that contains a loop of bowel that becomes strangulated - then death could be possible (esp if untreated).. But, garden variety hernias don't usually cause death. Please see doctor if you believe you have a hernia. Take care.

Yes: if it gets strangulated or causes rupture leading to peritonitis and sepsis. This will occur if the obvious symptoms are ignored

Yes: If the hernia becomes incarcerated it can strangle off a section of intestine causing it to die and leading to infection causing death.
