Infection or ?: a single white spot could be normal secretions that have formed a little seed of material, and are now stuck in the crypts in a normal tonsil. Multiple spots, along with a fever and pain, are often a response to a virus. Blotches of white to green goop that doesnt dislodge with gargling could be a sign of pathogenic bacteria, or (if you are immunosuppressed or taking steroids) monelia fungus.
