Lightheaded +vision: I've never met anyone with anxiety symptoms 24/7. In my experience, those symptoms tend to fluctuate. Have you considered having a hearing test, a neuro consultation, or an Opthalmology exam? Make sure you are eating a healthy diet and drinking adequate amounts of water to avoid dehydration and hypoglycemia as reasons.

Probably not: Usually when someone has an inner ear disease, they do not feel "lightheaded" but feel vertigo instead. Vertigo is described more like a spinning sensation where it is difficulty to maintain your balance or you may feel motion sick. Oftentimes it is triggered by certain positions of the head, and will calm down if you keep your head still and doesn't cause weird vision.
