?prostate: I would suspect prostate congestion or inflammation and/or infection. My recommendation is to see the urologist again for another evaluation.

Pain: I'm confused by your question. You are a "female" whose private part "stays up" at nigh with pain which wakes you up? are you a male with priapism? Females don't have this problem and they would see their Gyn. This may be neurologic or psychologic. Follow up with your doctor and please take the time to formulate your question more carefully so you can get a more useful response.
