Similar but: Both cause very sore throat with swelling and fever. Both can be diagnosed readily with a rapid test. Mono rapid test is less accurate. Mono significant systemic symptoms such as fatigue, sleepiness, nausea, and the swelling tends to be more severe and more dramatic appearing. Mono vs strep: Strep throat is a bacteria infection and usually responds with antibiotic treatment, penicillin or amoxicillin. It should be always treated if throat culture is positive. Mono is caused by a virus called Epstein Barr and often causes very enlarged tonsils and can not be treated with antibiotics. There is no antiviral available to treat this. It is associated with lymph node swelling and fatigue.
