IV contrast: is not required per se, both scans could be performed without contrast. However, depending on what your doctor is looking for, the contrast may increase the sensitivity of the scan for detecting certain subtle and/or specific abnormalities.

IV and imaging: The CT or MRI is much more sensitive for certain abnormalities that cause headaches if the test is done with IV contrast. Although it is not required. an IV helps make the study more useful to your physician.
