Is a cast of the forearm to the tip of the fingers appropriate treatment for a bone bruise for the metacarpals?

Depends: That is a fairly extensive cast for a "bone bruise" as you put it. A bruise does not necessarily have to be immobilized at all; but sometimes we will to control swelling, improve function, or we are concerned there may be more to the injury. Don't hesitate to discuss with your orthopedic surgeon.
