PRWP: PRWP or poor R wave progression can be a sign of a few things: most common problem I see is artifactual: simply a matter of suboptimal electrode placement of the EKG either due to sloppy technique or else breast tissue (in women). PRWP can also be a sign of a previous anterior wall MI or heart attack. Lastly, some conduction abnormalities such as left anterior fascicular block can cause this
