The "no acute ischemia" part means that there is no heart attack occurring at the moment. The rest really implies that the ECG tracing was not perfect in its appearance. This could be nothing, or it could be related to true heart disease. This type of EKG would need to be interpreted in context: symptoms, physical limitation, other testing.
