Poor R wave progression in an otherwise normal ECG is most often due to incorrect electrode placement. Despite it being a common "problem" , I am disheartened by the apparent lack of interest of individuals charged with the responsibility for technical quality of ECG recordings. This is a problem that causes some anxiety for the patient.
