Rearrange Support: By sleeping on your stomach for so long your body has become accustomed to this habitual position & with any habit, change can be challenging. Having good support such as arranging pillows around back/legs, etc. or a memory foam topper to mattress may give support/comfort to facilitate muscle relaxation & sleep. However if you are having breathing difficulty/short of breath, pain see your doctor.
