Infection.: Incontinence alone might be secondary to muscle weakness,. If fever with it probably has infection. Get it treated sooner, it can lead to a serious infection and other problems.

Exam: I would test your urine and also perform a complete history and physical. You could have an infection in your urinary stream. Please see your doctor

Infection?: A bladder infection is the first thing to come to mind. A urinalysis and culture are in order. In some cases, especially with fever, it might be good to begin an antibiotic while awaiting the culture result. Adjustments can be made when the results are back, but are often not needed.

Depends...: Does she have other medical problems? What are her vital signs? Is she coherent or does she have an altered mental status? Does she have a living will? The urinary overflow may or may not be related to the fever. She could have a FUO or urosepsis from a UTI due to urinary retention. Assuming she is full code, get her to the ER ASAP. She may need catheterization, antibiotics & a full sepsis workup
