Heart evaluation: You need to have a heart evaluation for possible ischemic heart disease and left ventricular dysfunction or valvular heart disease. If there is something with you heart it needs to be treated first.

See a cardiologist: An evaluation by a cardiologist is first; he/she may than be able to discuss your heart problems with the anesthesiologist who will work with your orthopedist or neurosurgeon to find a solution that will permit you to have surgery.
