Possibly...: The medical chart is not judgmental; it provides facts to document the reason for visit, findings, and treatment plan. A physician may find it important to document "wearing diaper" depending on the nature of your visit; however, please understand that this documentation would not be to place any judgment on you or your lifestyle.
