We would just: Be guessing but three possible immediately come to mind...the meningococcal, the T dap or the HPV. The best thing to do is call where you got them. They will have a record.

Contact your PCM: Your best bet is to contact your PCM/Pediatrics Physician from that time for copies of your Shot records. Sometimes your old school may have copies of Immunizations, but I'd recommend starting with the Physician who ordered the shots.
