More than likely: Research demonstrates that low back pain, particularly those related to disc issues, will have flare ups from time to time. Treatment should be thought of as management, rather than cure. The home exercise program prescribed should be thought of as a toll bag that you can return to in time of need, and more importnatly to prevent future flare ups. See your chiropractor and review exercise program.

Yes: This sounds like sciatic nerve irritation, probably at L4L5.This could be related to the herniated disc. It does not necessarily mean surgery but it does need follow-up and readjustment of the exercises if you are still doing them

Spine Pain Options: This pain in the distribution as you suggested is the result of an irritated nerve or facet joints or other injury typically in the lumbar spine (low back) which are caused by herniated disks, spinal stenosis or degenerative disc disease, etc requiring further evaluation by a spine specialist and may be candidate for facet injections/radiofrequency ablation and epidural steroid injection.
