Tonsils: Asymmetry of the tonsils does not indicate cancer. If you dtill have concern you might ask the ENT specialist to do a biopsy to put your mind at ease

Ear pain: It is very common for TMJ pain to be refered to the ear, it is also very common for your tonsils to be asymmetric ( one larger tan the other.) if you have been seen by your PCP and by an ENT I would think it unlikely to have the very rare tonsillar cancer. However, if you continue to be concerned, get another opinion by seeing another ENT.

Yes: If your only symptoms of "TMJ" is ear pain, I would worry. You can get referred ear pain from throat disorders, including cancer. I would get a second opinion by an ENT-Head & Neck surgeon at The University of Pittsburgh Department of Otolaryngology/Head and Neck Surgery.
