Bone spur: The extra prominence could be a bone spur coming from the navicular. This can be a normal occurrence given your foot type (flatfoot) or from an old injury. The best way to know for sure is from a X-ray. See your local podiatrist for evaluation.

Joint issue: A prominent navicular can be from either an enlargement of the bone or from a malalignment of your midtarsal joint. Usually it is a combination of both. The stress caused by the malaligment makes your tibialis posterior muscle pull extra hard and makes the bone bigger. I recommend you see a podiatrist and get xrays. At minimum you need orthotics but you may need surgery to correct this.
