Rarely: A small percentage of lupus patients are ANA negative. There are eleven diagnostic criteria which you can Google and find easily.

Very unlikely: 98-99% of people who have lupus will have a (+) ANA. If the ANA is (-) then the likelihood of lupus is very remote . In my experience the most helpful signs IF PRESENT are: 1) joint swelling (not pain) 2) A facial/body rash recognizable to someone with experience in recognizing Lupus rashes (there are many mimicks) 3) evidence of kidney disease ( by looking at urine tests) 4) low WBC or platlets.
