Differences: Pts with both diabetes out of control (DOC) and dehydration (D) have a deficit in blood volume. In dehydration, it is because the person is denied enough oral fluids to prevent it. In DOC, high levels of glucose in the blood causes an osmotic diuresis which makes the person lose large amounts of fluid. They both are seen with fluid volume deficits caused by different factors.

Easy to separate: If your blood sugar is too high you will piss out lots of body water drawn off by the sugar in your urine. This can and does cause dehydration. However, when a simple dipstick urine test shows all the sugar excess, you know it's diabetes. If the same urine dipstick test shows just concentrated pee & no sugar, you are simply dehydrated.
