Likely arthritis: Usually the problem with pain of the big toe is arthritis in the joint. The joint is unable to move effectively during walking/running. It is common in most people, but it is aggravated by overuse. A x-ray would be of great benefit to rule out other diagnoses, such as fracture or bone cancer. Treatment for arthritis, includes cortisone injections, mobilization exercises, orthotics and surgery.
