Angio: Since you know her dorsalis pedis artery is clogged, i assume she has already had an angiogram (invasive or non-invasive). An expert in peripheral revascularization (vascular surgeon, interventional radiologist, or cardiologist with peripheral training) needs to review the angio to see what options there are for treatment.

tough problem: Sounds like she has tibial (calf) vessel atherosclerosis and may have reduced circulation to the toe. This may be amenable to treatment with catheter based therapy or surgery. She should see a vascular surgeon for evaluation.
