A member asked:

Apart from myeloma, can mgus be found in urine only (upep) with a normal spep? or does it always show in blood?

A doctor has provided 1 answer
Dr. Ronald Krauser answered

No to 1, yes to 2: It always will be present in serum if it is truly present.

I had a 24 hour Urine Protein Electrophoresis Test. My total Protein was 4 mg/ dL. UAlbumin 0.6, UAphla 1 0.5, UAlpha 2 0.6, UBeta, 0.7, and UGamma 1.7. No M spike observed. Are these normal ranges? Does this rule out Myeloma and Amyloidsis?

This is for my mother, aged 55. High serum kappas (33), normal lambdas and marginally high kappa lambda ratio (1.9). SPEP and Immunofixation show polyclonal gammopathy (in IgG Kappa and Lambda). Should we be concerned about MGUS/Myeloma ?
Serum free light chain test shows normal kappa (8.7mg/dl), low lambda (3.80), high kap/lam ratio (2.31). Serum $ urine electrophoresis show no monoclonal protein, nor does serum immunofixation. Likely MGUS, myeloma, or admyloidosis? CBC/met panel ok.
I have inconsistent tests that lean toward multiple myeloma or MGUS. First xray lytic lesion; globulin series serum & urine monoclonal positive to gamma; 24 hr electopherurine neg; beginning anemia; BUN off; other small abnormals. Bone survey neg.
Is there any special blood test to find out mgus or smoldering myloma?
Hello. My neurologist ordered the following tests in 2018. Normal results except for high kappa light chains. Ratio normal Why is it high? Am I at risk for mgus or myeloma? Immune profile and cpk normal. Retest? Kappa0.03-1.94mg/dl My result 2.65?
