Devel vs. infection: Could be a problem in development or could be a physiological problem. First you want a family history as to the ages when both parents were stool controlled at night. If they were late then that could be the problem. It could also be a problem with intestinal parasites -i.e. pinworms. Check at night and if positive easily treated with a single dose of mebendazole.

Time for consult: I would have him seen by his PCP and evaluated for chronic constipation/stool retention. If your PCP is unfamiliar with the issue, contact a pediatric Gastroenterologist or developmental pediatrician.The various problems that lead up to this symptom are complex, and often take months and years to resolve with proper help.
